Application Forms
Municipal Road Damage Deposit
The Municipal Road Damage Deposit form is required for all exterior construction projects, with the exception of Residential FASTRACK applications or developments under Site Plan approval by City Planning. This deposit acts as a financial guarantee to protect the city’s infrastructure from any potential damage caused during construction, including curbs, sidewalks, roads, laneways, and boulevards.
It also addresses issues like mud tracking and failure to clear snow or ice from sidewalks. Once construction is complete, the deposit is handled and potentially refunded by the Right-of-Way Management team in the Transportation Division.
Commitment to General Reviews
The Commitment to General Reviews form is required for buildings that need professional design and field review under the Ontario Building Code. It must be signed by the owner (or agent) and the responsible architect, engineer, or consultant.
Schedule 1: Designer Information
This form is required for all designers applying for a Construction Permit, excluding architects and engineers. Property owners claiming exemption from qualification or certification must complete this form to state the reason for their exemption.
Declaration of Use
The Declaration of Use form may be requested by an examiner to clarify how a building will be used. If required, the registered property owner must complete and sign the form, clearly stating the proposed use of the property.
Zoning Applicable Law Certificate Application Form
The Zoning Applicable Law Certificate Application Form is used for a preliminary review of plans to ensure zoning and legal compliance, facilitating a complete building permit application. It is strongly recommended for Committee of Adjustment Applications and requires a full set of architectural plans.
Use of Zoning Applicable Law Certificate
The Use of Zoning Applicable Law Certificate form must be completed by the owner when submitting a Zoning Applicable Law Certificate as part of a complete building permit application. This declaration confirms the intended use of the certificate.
Infill Public Notice Declaration Form
The form is required to request an electronic copy of the Residential Infill Public Notice from Toronto Building. It must be submitted by the owner or their authorized agent.
Plumbing Data Sheet
The Plumbing Data Sheet is required when applying for a plumbing permit for plumbing work on private property or within buildings and structures. It provides essential details for the review and approval of plumbing-related projects.
Owner’s Acknowledgement of Demolition Control Conditions
This is required when applying for a Demolition Permit to demolish a residential building or unit. It ensures the owner acknowledges the conditions set for the demolition process.
Tree Declaration
The Tree Protection By-law Declaration form is required for projects that may impact private or City trees. The applicant must also comply with additional tree protection bylaws, including Tree and Ravine Protection, to prevent damage to trees during construction.
Assumption of Responsibility for Engineering Content
The Assumption of Responsibility for Engineering Content form is required when permit drawings for a new house or alterations bear a professional engineer’s seal. The engineer must confirm their responsibility for the content by completing this form.
Application for a Permit to Construct or Demolish
The Application for a Permit to Construct or Demolish is required when applying for construction or demolition permits, including swimming pool enclosures and sign applications. Please refer to Schedule D of the City of Toronto Municipal Code, Chapter 363 for specific drawings and submission requirements. Additionally, check the fee schedule for the current applicable fee. Mechanical and plumbing applications related to a building permit are considered part of the same project and their fees are included in the building permit, but the building permit must be applied for first or simultaneously. When applying, include the (9-digit) project number on the application. For multiple discipline applications, select the appropriate combination in the description box on the form.
Building Design Information Form
The Building Design Information form is required for all new buildings and additions to ensure compliance with the Ontario Building Code. The Ontario Architects Association (OAA) Matrix may be used as an alternative.
Worksheet for Evaluating Alternative Solutions
The Worksheet for Evaluating Alternative Solutions is required when applying for an alternative solution evaluation under the objective-based code. Attach all relevant reference documents to this form and submit your application as a single PDF. Only a qualified designer may apply.
Energy Efficiency Design Summary
Required when applying to build a new Part 9 residential building or add to an existing one, ensuring compliance with the energy efficiency standards set out in Supplemental Standard SB-12 of the Ontario Building Code.
